Bristol Royal Hospital for Children – This ‘Trust’ (who you should NOT ‘Trust’, have been deceitful, despicable liars. God help me for saying this but I wouldn’t shed a tear if each and every one suffer a similar trauma. The one’s who passed by looking down their noses outside the hospital are all party to the cover-up.People who either caused, were involved in or were complicit in the deliberate cover up of baby Ben Condon’s death. Yesterday the Parliamentary Health Service Ombudsman (PHSO) issued a report after 4 years of investigating that found around 100 failing in Ben’s case, yes that is 100 failings in one case in one hospital. Just imagine how many more there are under that roof. There were so many failings and attempts to deceive that the PHSO said in its report ‘It is Impossible to know whether steps taken by Trust will prevent the same things happening again.’ In 2017 two and a half years after Ben died the hospital finally admitted publicly that they had failed to give antibiotics and that this had contributed to Ben’s death. In a private statement to the Secretary of State they said ‘had we given Ben antibiotics earlier, on the balance of probabilities he would have survived’ The PHSO agrees that antibiotics should have been given earlier but finds that this was just one of many failings which reduced Ben’s chance of living. The minute Ben stepped in to that hospital his life was over. He stood no chance. ‘The Trust has failed to be open and honest with the family about the events surrounding Ben’s death. It has done this to such a degree that it could be seen as a deliberate attempt to deceive.’ ‘It is entirely understandable from the evidence we have seen how the family have reached the view that staff conspired to cover up details of Ben’s death’ ‘The repetition of this failing across medical and nursing staff suggests to us an organisational failing on the part of the Trust and a failure to provide effective services to Ben and to Mr and Mrs Condon.’ The most irreparable of all failings was that the Trust waited until Ben had been cremated before telling us 7 weeks later that he had an infection which they had failed to treat. They never logged the infection in the medical notes, lied about test results, when tests had never been taken, hid test results for over a year and waited until Ben was in the ground before issuing a death summary 3 months later. Doctors said they did not think it was appropriate to speak with us about a post mortem after Ben died. Of course they didn’t this was just their way of burying their mess. The report highlights a universal failure to be open and honest, 4 potential breaches of the Duty of Candour, a very significant failing, a remarkable failing, 10 occasions of a significant injustice, a failure to adhere to the national framework, a breach of the serious incident framework, Failure to carry out a serious investigation or Root Cause Analysis and 2 occasions of service failure. The report also highlights 6 failures to meet the standard set out in the NMC ‘The Code’, 10 failures to act in accordance with GMC ‘Good Medical Practice’, 9 occasions of a failure of NHS Constitution, 8 occasions of a failure to adhere to Ombudsman’s Principles and 6 failings of GMC Guidance. In addition, the report highlights several occasions of failing to follow hospital protocol, 2 occasions of failing to comply with relevant standards, Good Medical Practice and The Guidance and further occasions of failing to adhere to ‘The Code’, PICS Guidance, Criteria for Hospital Post-mortem and also for failing to comply with the recommendations of Verita following an independent investigation. Staff negligently failed Ben, they lied to cover up, they hid evidence, they were caught trying to delete documents and recorded evidence and they are protected by the regulators who sit back and do nothing.Bristol City Council, Bristol Major, Bristol MP’s all take a bow. You were all made aware of this and did nothing about it. They lied at an inquest in 2016 to cover failings and these exact Doctors still treat in PICU to this day.
